The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.
We do not double book our schedule. We appoint plenty of time for each procedure and your appointment has been reserved especially for you. We request at least 24 hours notice if you need to cancel your appointment. After the second time of missing your appointment without notifying us in advance, you may be charged an additional fee.
If you are a new patient to our office, please visit our patient forms page to download our new patient forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. We accept payments by way of cash, Check, Money Order, Visa, Mastercard. We have dental loans available through Citi Health Cards, Lending Club, and Care Credit – We can offer our patients, upon credit approval, an interest-free term loan (up to 12 months) with no down payment, no annual fee, and no prepayment penalty. Ask for an application- we can help you fill out an application and file it for you, giving you an immediate credit decision.
Insurance offers a practical means of making your health care more affordable. Accordingly, we are happy to accept those insurance plans which allow you to choose which dentist provides your dental care and makes the adequate payment for the dental plan. We encourage you to discuss your dental plan with our Office Manager, who can see if it’s a plan we accept.
We are a Dental Provider to PPO plans like Aetna, Ameritas, Assurant, Cigna, Delta Dental, Fortis, Metlife, Premier, and United Concordia, United Healthcare, Healthnet, Principal, Dental Benefits Provider, Blue Shield, Blue Cross. We recognize most insurance companies as an “Out-of-Network” Provider.
We are committed to helping you use your insurance benefits to provide the best care possible. Due to the complexities of the insurance contracts, we can only estimate your coverage in good faith but cannot guarantee coverage. The estimated co-pay amount is due at the time of service. Our office will bill the insurance company directly and allow them 30 days to render payment. Most payments are received within this time. After 30 days, the patient is responsible for the balance to be paid- in- full. If you have any questions, please call CHRISTINA at 916-972-1933 and she will be happy to help you.
